Hospice Team

Our interdisciplinary team is composed of skilled and highly trained professionals and volunteers. We prioritize patient and family needs first.

• MEDICAL DIRECTOR collaborates with the patient’s primary physician and is available for pain and symptom management consultation and also makes house calls, as needed.
• The DIRECTOR OF NURSING is a skilled nurse who supervises the day-to-day medical needs, progress, and care of the patients. SKILLED NURSES provide regular visits and provide direct care to our patients. SOCIAL
• WORKERS are compassionate and trained to provide emotional support. They help with navigation and connection to resources.
• HOME HEALTH AIDES help with personal care such as bathing and hygiene.
• CHAPLAINS offer spiritual and emotional support throughout the end-of-life process and are available post-death to provide bereavement processes.
• VOLUNTEERS are passionate individuals who offer companionship.
